Dr. Stoll: Liver question

Hi Dr. Stoll,

I am experiencing bile salt diarrhea thanks to the bad decision to have my gallbladder out in January of this year. In your experience and expertise, do you know if the liver can adjust its bile flow over time to produce less? Will eating less fat (but enough for health) "train" the liver?

Currently I'm trying supplements and dietary changes to accommodate my liver's bile production schedule, like taking Caltrate D-600 (0.5 tablet before a meal 3X per day) and psyllium fiber in water (2x/day), and modifying my diet to be more fibrous, lower in fat, and eating more often.

Hi, CAC.

Physiologically, there is no relationship between the production of bile from the liver and the presence or absence of the gallbladder. The galbladder exists only for the purpose of giving an extra "shot" of concentrated bile when one overdoes the intake of high fatty foods. The production of bile by the liver is a steady stream pretty much no matter what you do.

Isn't it more desireable to bind the bile and divert it through the colon instead of allowing it to be reabsorbed
and cause a rise in cholesterol?

Thanks for the information. I've had two week-long bouts of diarrhea since surgery in January, and I'm currently in the second. I went a few months with no problems but now am in a chronic state of yellow diarrhea. I suspect that it was because my diet was too high in fat recently, and I'm hoping with diet modification that it will revert back to the healthier condition I've experienced between bouts. I've got my fingers crossed that it will.

Yes, indeed. I am eating much smaller portions and better quality food, though the introduction of bile salt diarrhea makes any eating kind of a chore...

Any dietary suggestions? I've read that lecithin is pretty good.

I use lecithin all the time. It's an emulsifier so I guess it would assist the bile but I wonder why you are producing so much bile. I wonder if eating more healthy fats would help.

Thanks for your response. I suspect I may be producing so much bile all of a sudden because I was overeating. Shamefully, I binge eat, but frankly, after this last week's episodes of incontinence from bile salt "d" I will not do it again. I've got my fingers crossed that my body will adjust back to normal, and I've signed on with a counselor who specializes in stress eating, so I'm where I need to be.

Yes, I am trying meditation (I have a beginner's meditation CD made for me by a friend), a little bit of progressive relaxation (tightening and then releasing certain muscle groups), and some prayer.

What kind of SR practice do you find is the best for you?

Hi CAC, I found this information about leaky gut very interesting regarding bile.

For example, the body creates bile not only as a lubricant to flush wastes out of the liver, but also, by its cold and bitter nature, to detoxify many of the poisons accumulating in the liver. Bile however is extremely caustic to large intestine epithelium. When bile enters the small intestine via the common bile duct, beneficial bacteria break the bile salts down into a less caustic compound, making it safe by the time it reaches the large intestine. Antibiotics destroy these bacteria and thus allow bile salts to enter and damage the large intestine. Perhaps this may explain the high incidence of colon cancer.
