Do I really need a surgery?

Hello Sir,

This site has been pretty useful for people like me. It has given a lot of information on the PC and how to avoid the flare-ups.

Three months ago, I noticed blood coming out of on the top left side of my buttock crease. This was on my flight journey of about 24 hours. After cleaning the same, I noticed that there was a small hole out of which the blood was coming out. This hole has not yet closed by itself. Hence, I consulted a GP who forwarded me to a Surgeon. During these three months, little blood comes out on some days but on other days it is perfectly fine. Also, I have noticed that the blood starts coming out when I wear tight clothes (the reasoning for which is given on this site). Also, I dont have any pain around that area. The surgeon did ask me to clean my hair and then re-visit again after three weeks to see if the opening has closed (along with an antibiotic). But the hole has not closed. Now he is recommending a surgery wherein he is going to cut it open, put some blue ink like material into it to see where is the root, and then clean up the entire stuff. He has said the healing will take about a week or two.

It will be greatly helpful if you could answer these questions:
1. Is surgery required in my case? (since there is not much pain)
2. If I dont go for surgery, can I keep the situation in control by using the techniques mentioned here?
3. The Doctor said that if I dont do the surgery, it will aggrevate within. Is that true?

In my opinion, if you entrusted your "butt" to the doc who has been giving you advice, you would be making a big mistake. First of all, even from here I can make the diagnosis of pilonidal sinus. He couldn't even make it from there. IF he actually did make the diagnosis, his initial management was totally inept.

The "dye thing" is exactly what we used to do 40 years ago--I assisted in a number of those surgeries, myself. If I had this I would first give a good try at the stuff described in the archives and try to live with it. The most effective thing would likely be getting into the habit of carrying around a donut to use whenever you sit down. If that is too embarrassing, get a good inflatable one and carry it in your pocket. Blow it up whenever you have to sit down.

In the meantime, in my opinion, remembering that "To a hammer everything looks like a nail.", find yourself a much better doc who will give you alternatives to definitive surgery.

Putting off surgery will not cause this to progress or be worse whereas the surgery has about a 30% risk of making things worse and only a 50% chance of resolving the problem.

Thanks very much for your reply. I am onto finding another Doctor now. I will keep you posted how it goes.

Btw, is the dye thing still being used? Is it effective? And the Doctor said I will be back to work in about 10 days after surgery. Is that true?

Yes, Raj, the "dye thing" is still being used but is of limited effectiveness.

Raj when you go to a new doc - Ask if it could be a Fistula ???? They can be on the outside of the body in the butt cheek area as a small abscess = quite painful at times. Yet it is deep into the body.

Ask if its not brought up in discussion.

I have had 17 removed to date via Surgery. All started as a small lump that turned painful within the rectal area/butt cheek ... some small, some much bigger.

Google "Fistula" and you will understand. It is a cyst Yes and can be confused with a Pilonidal cyst.
