Pilondial cysts surgery wound not healing

My son had surgery for his pilondial cysts in 2/2009, the surgeon said he had sinues that ran the entire length of his buttocks. The incision is at least 7 inches long, basically from the base of spine to anus. The incision began to come open after stitches were removed. The wound is now completely open and he has been going for the last 6 months to have laser hair removal , which is very painful and the wound is not showing any signs of healing that I can detect, the plastic surgeon says it is healing. I am on my second jar of 1% silver sulfadiazine cream which we apply daily to the wound, it is left open no packing and he is forced to wear a sanitary napkin as to not get cream on clothing. I truly wish that I had found your site prior to get the surgery. Upon reading your site, I am inclined to beleive the plastic surgeon on the hair removal as a possible solution. Was wondering if you have an advice on any other salves or treatments to help heal this wound. Needless to say this whole incident has taken its toll on my son emotionally and socially. I am trying to be positive about the whole ordeal but am beginning now to wonder if the orginal surgeon just complicated and created the situation we have now. I would appreciate any help or advice you can offer at this point. Thanks

BOY! RC.

Your son is now in a terrible and probably irreversible position, in my opinion. "Hindsight is always better than foresight." I just wish surgeons would be more informative of the risks of surgery for this problem than they tend to be--before the surgery.

I donot know what else he can do now but follow the path outlined by the surgeons. Listen to carolB and do not stop learning. Please share what you learn and how it works.

I have a dog with perianal fistulas, which is similar.

They no longer recommend surgery for these (in dogs at least). They are typically treated by fixing the diet (for dogs, single protein and single carb to ensure no allergens), antibiotics for secondary infections, immune suppressants for the immune system contribution (pred or cyclosporin), and a topical (tacrolimus).

For my dog, he only heals when he is on a combination of 3 antibiotics(I think there is a tick diseaese connection in his case), I have had him on no suppressants, use the topical tacrolimus, but also do the following....

Use a variety of homeopathic meds depending on symptoms. Lachesis (shooting pains), hypericum (nerve pain), arnica (tissue healing and muscle pain). On the open wound, I only have access through an abcess hole, I insert a combination of aloe vera gel mixed with potassium iodide as topical disinfectants. Other times I put in liquid vitamin E (Solary and Country Life make some nice liquid vit E products). Those last two make him much more comfortable.

I had acupuncture done on the area once, and he responded very well, I haven't had time or $$$ to take him back, but I would recommend it, and will be taking him back. I have a photonic light unit here at home. It helps bring blood flow to the area, needed for healing.

I have tried backing off the antibiotics, but within four days, the wound starts getting worse, and eventually very bloody, and then opens more. He is on tylan for his intestinal issues (and also for staph), amoxicillin (baytril/cipro also worked, but it scared me), doxycycline, and also the antifungal ketaconazole (because GSDs are very prone to fungal issues, and he did have them). I think that he might have been healed by now, but we kept trying to take meds away, which caused a setback each time.

Weaning him off the meds will be tricky, but one step at a time....
